Community Health Assessment

Since the late 1980s a variety of Allen County agencies have prepared and administered individual needs assessments and community health assessments. This report marks the third community assessment undertaken by a broad group of community agencies, with the first being completed in 2002. Over the years, these assessments have shown that individual health is closely linked to community health.

Health AssessmentCommunity Health Improvement Plan

(CHIP) defines the vision for the health of the community. It is a long-term, systematic process of addressing health issues based on the results of assessment activities. This plan is used by health and other governmental, educational and human service agencies along with community partners, to set priorities and coordinate and target their resources. A CHIP is critical for developing policies and defining actions that promote health.
